They often grow in tilted or stay buried in the gum, which can trap food, cause decay or gum inflammation, and damage the tooth in front.
Does every wisdom tooth need to come out?
No. Removal is considered when a wisdom tooth:
- is tilted or impacted so food gets trapped and it cannot be cleaned
- has decay, or has caused decay in the molar in front
- causes repeated gum swelling and pain
- pushes on the teeth in front and affects alignment
A wisdom tooth that grew in straight, works for chewing, and can be kept clean does not have to be removed.
Why CT before extraction
Lower wisdom teeth can sit close to the inferior alveolar nerve. Panoramic and CT images show the 3D relationship between the roots and the nerve, so the extraction can be planned around it. When a case is high-risk, the dentist will explain this openly and, if needed, refer you to an oral and maxillofacial surgery department.
After the extraction
- Pain usually starts 2 to 4 hours after surgery as the anesthetic wears off, and peaks around day 2 to 3; take the prescribed painkiller before the numbness fades
- Swelling peaks around 48 hours; a cold pack helps during that time
- On the day of surgery avoid straws, hard rinsing, and frequent spitting so the blood clot stays in place
- Avoid smoking and alcohol for at least 2 to 3 days
- If pain gets worse 3 to 4 days later, or there is a bad smell, come in to check for a dry socket
If you are flying soon after, mention your travel dates when booking. Stitches, when used, are usually removed after several days.
